The 5210 Jefferson County initiative was launched to increase physical activity and healthy eating to reduce chronic disease in Jefferson County. The initiative addresses strategic priorities identified in the 2016 Community Health Improvement Plan.  5210 Jefferson has representation from multiple sectors:  schools, afterschool, early childhood, workplace, healthcare, and community.

 

The vision of 5210 Jefferson is to create healthier communities by promoting healthy lifestyle choices. Modeled after a very successful community-wide program in Portland, Maine (Let's Go! Maine), our mission is to increase physical activity and healthy eating through lifestyle and community changes to improve the health of Jefferson County residents.
