5210 Jefferson

The 5210 Jefferson County initiative was launched to increase physical activity and healthy eating to reduce chronic disease in Jefferson County. The initiative addresses strategic priorities identified in the 2016 Community Health Improvement Plan (PDF).
5210 Jefferson has representation from multiple sectors: schools, after school, early childhood, workplace, healthcare, and community.

Vision & Mission

The vision of 5210 Jefferson is to create healthier communities by promoting healthy lifestyle choices. Modeled after a very successful community-wide program in Portland, Maine (Let's Go! Maine), our mission is to increase physical activity and healthy eating through lifestyle and community changes to improve the health of Jefferson County residents.

The 5210 behaviors are based on science and recommended by the medical community to promote good health.

5210 Jefferson is working to implement 5-2-1-0 in homes, schools, workplaces, and healthcare and childcare settings. This broad approach makes it easier for all ages to adopt it, wherever they live, learn, work, or play.
